*Hotel Reservation form*

1. Please fill the reservation form and send it by fax and email directly to the hotel. All corresponding fax numbers and email address are listed in the reservation forms.

2. Authors or participants will be confirmed directly from the hotels about their reservation requests, not from the conference.

3. If a requested room type is unavailable, please re-select another available room type.

4. When you
1) send the reservation form;
2) ask any question related to a hotel, such as room booking;
3) confirm the reservation.
